Podiatry

• Podiatric licenses expire on September 30 of even years.
• At least 60 days prior to the expiration date of a license, DOPL will email a renewal notice to the licensee's email address of record as provided to DOPL by the individual licensee. (Failure to notify DOPL of an address change could prevent a licensee from timely receiving renewal notification and other important correspondence from DOPL.)

The following requirements must be met:

• Podiatric Physician: Complete the required hours of CE as indicated in R156-5a-304
  Controlled Substance: Complete the required hours of CE as indicated in 58-37-6.5
• Complete at least ONE of the following online suicide prevention trainings (these qualify for CE credit):
  • Suicide Safety Planning with Patients (0.5 credit)  
  • Talking to Patients About Suicide (0.5 credit)
  • Counseling on Access to Lethal Means (0.5 credit)
  • Suicide Prevention (0.5 credit)
  • Supporting Patients (0.5 credit)
  • Collaborative Safety Planning (0.5 credit)
  • CALM-Utah (Counseling on Access to Lethal Means) (1 credit)
