Apply for a Marriage and Family Therapist License (Standard Application)

The following requirements must be met:

Verification of Post-Graduate Supervised Experience. NOTE: You must have each supervisor complete a separate form, and the hours from all forms must total 3,000.
Official score report of passing the AMFTRB Exam
