If you are currently licensed as a Clinical Mental Health Counselor in a state, territory, or district of the United States you may qualify for licensure by endorsement. Click here for additional information.

If applying for Initial Licensure and you do not currently hold a Utah ACMHC, you must also provide documentation of meeting the education requirement (submit one of the options below):
- Master’s or Doctorate degree in Mental Health Counseling program ONLY that is accredited by CACREP. Submit OFFICIAL transcripts that identify the degree earned and date conferred.
- OR - - Master’s or Doctorate degree in Mental Health Counseling program or equivalent field that is accredited by CHEA. Submit OFFICIAL transcripts that identify the degree earned and date conferred. Submit Course descriptions or syllabus for EACH course Complete and submit the EDUCATIONAL COURSE REQUIREMENTS pages.
The following requirements must be met:
- Supervision for Post Graduate Mental Health Practice Hours. NOTE: The hours from all forms must total 3,000.
- Official score report of passing the NCMHCE and NCE.
