Under Utah Code 78B-5-618, healthcare providers using third-party services for patient medical record access must submit contact details for both themselves and the third-party to DOPL. Required information includes provider and third-party names, contact numbers, fax, email, website, and mailing address. Providers are responsible for keeping these details updated.
