Yes, your application will only be held in an incomplete status for 30 days. If you will not have all items submitted to DOPL within 30 days, your application will be denied and you will be required to submit new application, fees and all supporting documents.
Enclose with the application complete information with respect to all circumstances and the final result for EACH and EVERY incident:
- Personal account of the incident
- Police report(s)-You will need to contact the police department that responded to the incident
- Court record(s)-in the local jurisdiction where the incident took place.

If you are unable to obtain any of the records required above, you must submit documentation on official letterhead from the police department and/or court indicating that the information is no longer available.
You must submit: a renewal form; renewal fees; a current Certified Professional Midwife certificate in good standing with NARM; a current certification in adult and infant CPR; a current certification in newborn or neonatal resuscitation; and documentation of completing the 2 hour approved CE requirement in intrapartum fetal monitoring.
Renewal information is disseminated to each licensee at the licensee's last known address, as provided to DOPL, approximately two months prior to the expiration date shown on the license. Once you have been notified by DOPL you can renew your license.
"Approved continuing education" means: continuing education that has been approved by a nationally recognized professional organization that approves health related continuing education; a course offered by a post-secondary education institution that is accredited by an accrediting board recognized by the U.S. Department of Education, an MEAC approved midwifery program or accredited midwifery school, or an MEAC approved program or course; or continuing education that is sponsored or presented by MANA or any subgroup thereof, a government agency, a recognized direct-entry midwifery or health care association.
All licenses in this profession expire as a group on September 30 of even years. Therefore, the length of a licensee's first renewal cycle depends on how far into the current renewal cycle initial licensure was obtained. Licensees that were licensed during the last 12 months of a renewal cycle shall be licensed for a full renewal cycle plus the period of time remaining until the impending renewal date. Each renewal cycle thereafter is for a full two years. Each licensee is responsible to renew licensure prior to the expiration date shown on the current license. Renewal information is disseminated to each licensee at the licensee's last known address, as provided to DOPL, approximately two months prior to the expiration date shown on the license.
